Your personal details (email) will not be attached to your responses, will not be provided to DVA, and will not be stored by ORIMA after the survey period has ended.

If you provide free-text responses that contain a reference to self-harm, suicide, or threats to the health or safety of others ORIMA staff may be obliged to provide your details to the appropriate authorities.
